The MedServ Group’s optometrists provide comprehensive eye care for long-term care residents. Patients receive customized treatment, including regular exams and after-hours care, to promote good visual function and eye health.

Why Eye Health Matters

Eye health is very important for residents’ well-being. Many residents have limited mobility, so they spend hours reading, watching TV, working on a computer or doing other activities that require them to use their eyes. 

Residents with serious eye conditions have a hard time with their daily routine, which impacts their physical and mental health and requires more resource-intensive care. The MedServ Group provides optometry services that meet patients’ needs, so they can conduct their ADLs (activities of daily living).

  • On-site pre-and post-operative care

  • Completion of minimum data set (MDS) Vision Portion Section B

  • Comprehensive dilated eye exams

Our optometrists ensure that each resident can see their best by addressing both acute and chronic conditions. Highlights of our program include:

 
  • Optometrist Pratik N. Vyas, OD, is our clinical director. With nearly a decade of experience, he leads our team of skilled practitioners, including optometrists, opticians and nurse practitioners.

    Our optometry providers have more than 15 years of experience, on average. They are all accredited by the National Board of Optometry and by the governing optometric association of each state they practice in.

    The MedServ Group providers also complete ongoing training and education to stay on the forefront of their field and bring the latest evidence-based treatments to their patients.

  • The MedServ Group optometry providers have expertise in the specific eye care needs of older patients. We often treat residents with medical conditions, such as diabetes or high blood pressure, that can affect eye health.

    Our providers also understand how to connect with patients who have cognitive decline and may not cooperate easily with treatment. We know how to assess and accurately diagnose patients who are nonverbal or noncompliant, based on our experience with these patients.

  • Our optometry providers bring their staff and mobile equipment directly to your residents, so they receive thorough, comprehensive care in their own rooms or other preferred locations. Staying on-site is safer and more comfortable for residents who have vision loss, or are having their eyes dilated for exams.

    The MedServ Group’s optometry team diagnoses and treats all types of diseases and conditions affecting the eyes. We can handle most procedures at your facility. Our team provides all follow-ups and referrals as needed to guarantee proper management and continuity of care.

    We can also consult our other specialists at The MedServ Group, as necessary. For example, we may refer patients to our dermatologist to assess skin tags around the eye.

  • Eye health can be a window into a patient’s overall well-being. Our providers take a holistic approach to patient care, based on each resident’s medical history and related conditions.

    For example, diabetes and high blood pressure can trigger serious eye conditions, including problems caused by damaged blood vessels in the eye (retinopathy). Our team customizes a treatment plan to treat these eye conditions and minimize other health risks.

  • Many patients in long-term care facilities have other conditions (comorbidities). Our care plans require close coordination with other providers treating those patients. This includes communicating with other providers before prescribing some medications, to prevent adverse reactions.

    We work with other specialists to develop customized care plans for each patient’s specific needs, so our treatments work together in harmony with the patient’s body.

  • It’s best to be proactive when caring for vulnerable patients. Our providers detect, prevent and treat eye conditions such as glaucoma, macular degeneration and retinopathy, which can lead to blindness. These conditions can dramatically reduce a patient’s quality of life if left untreated.

    We use a mobile retinal camera to see inside the eye and diagnose any issues. We also provide ongoing follow-up to monitor eye conditions and track each patient’s eye health over time.
